Please note we need to know your biological sex for room allocation purposes. By law we can only allocate shared rooms to people of the same biological gender.
We will usually contact the parent/guardian instead of the young person, but on occasion it may be necessary to contact the young person directly.

Terms
Travel & Insurance
Travel to Lourdes will be by coach. The total cost of the Youth Pilgrimage is £720.
This will include: travel insurance, full-board accommodation in Lourdes, 2 polo shirts. (Not included are cancellation
insurance and any meals taken on the journey.) You could reduce this cost by fund-raising, so find out what events you
could hold in your Parish, school or college.
It is highly recommended that individuals take out their own travel insurance.
For those travelling with the Youth Pilgrimage, the organisers have a duty of care, for standards to be maintained, ALL who go as part of
the Youth Pilgrimage need to accept and comply at all times with the commitment specified on the next page.
Safeguarding
I am committed to protecting and safeguarding children, young people and vulnerable adults from abuse.
I understand that to knowingly give false information or to omit information will be considered as a breach of
trust and could result in the cancellation of any agreements made.
I declare that any information I have given in this application is correct.
I understand that if I am subsequently convicted of any criminal offence, I must declare this to the Director of
the Youth Pilgrimage.
I also understand that this position may require an enhanced DBS disclosure, and those details will be entered on
a national Safeguarding confidential database and retained indefinitely.
Commitment
I acknowledge that I have read, or can access at a later date, the Code of Conduct for all Liverpool Archdiocesan
Pilgrims contained in the Rights, Roles and Responsibilities document.
Where the Liverpool Archdiocese Youth Pilgrimage feels it necessary, failure to meet the obligations of the Youth
Pilgrimage Safeguarding Policy and/or Youth Pilgrimage Code of Conduct and/or Youth Pilgrimage Commitment may lead
to you being placed on the Pilgrimage Guidance Pathway.
Passports and Visas
I acknowledge it is my responsibility to check my passport expiry date, my EU entry/exit status (ETIAS),
and if I need a visa before travel to France; and accept that refunds cannot be given for failure to check
passport, ETIAS and visa status.
GDPR / Data Protection Act 2018
In accordance with the Data Protection Act 2018, I give my consent for any information contained in this
application to be processed and stored. The information provided is being used for the administration of the
Youth Pilgrimage. It is considered the legitimate interest of the organisation and is necessary for compliance.
Use of your information
Information provide in this application will be held for the pilgrimage year (which ends on the day new application
forms go online for the succeeding pilgrimage).
Contact information will be shared with the coach leader for the coach of which you have applied. And may be
shared with other parties associated with the Lourdes Youth Pilgrimage and relevant and appropriate diocesan
bodies and authorities.
Young people that require DBS certificates may be contacted by the diocesan safeguarding team through the
year to ensure the form is complete before travel. This could be during exam time and is unavoidable if
forms are to be processed before summer.

The payment schedule is Payment 1 £210 by 1st February – Payment 2 £210 by 1st April – Payment 3 £200 by 1st June (If you applied before the 20th October and qualify for the early bird price the £50 reduction will be taken from this final payment) . Payment details will be on your confirmation letter. The option for regular standing orders is also available – please contact the pilgrimage office for more details.
